I give permission for the coaches to administer first aid in the event of injury and to contact the appropriate emergency contact/s and /or services. *
I declare that my child is healthy and fit for the proposed activities, and I will notify Ready 'Net' Go if this changes. *

I intend to pay via; (spaces are limited and fill up quickly, pre-payment is recommended) *
Payment in full is required to secure your daughter's place and is only confirmed once payment has been received. Payment is non-refundable.
